Day Shift - 7.5 Hours (United States of America) Job Summary: The Social Worker will provide clinical and case management care to patients with intellectual and or developmental disabilities across the life span in an outpatient setting. The clinician will also work with children ages 0-5 in our Early Childhood program. Documentation must be in a timely and quality manner. Conduct individual, group, and family therapy. Assess, identify, and provide services for patients/families; address psychosocial concerns as well as post hospital care, including crisis intervention, advocacy and community referrals. Requirements

  • Master's degree required.
  • NYS Registration/Certification as a Social Worker (LMSW/LCSW) is required.
  • Experience with the assigned patient population is required.
  • Skilled in psychosocial assessments and delivering evidence-based therapy.
  • Able to intervene in mental health or situational crises.
  • Experienced in connecting clients to services and managing treatment plans.
  • Committed to equitable, client-centered care across diverse populations.
  • Strong documentation, collaboration, and interpersonal skills.
  • Knowledge of HIPAA, NASW Code of Ethics, and legal reporting requirements.
  • Understanding of DSM-5, trauma-informed care, and co-occurring disorders.

Nice To Haves

  • 2 yrs post-graduate work preferred.
  • Prior experience in a hospital or community health agency is preferred.
  • Additional language fluency and computer literacy preferred.
