About The Position

To provide documentation for discharge planning, psychosocial, treatment plan, UR review, and care coordination for patients. Intake phone calls and assessment patient for appropriate programs.

Requirements

  • Bachelor's Degree in Social Work.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
  • Good customer service skills and experience working with diverse population.

Responsibilities

  • Documents all discharge planning and progress towards goals.
  • Consult with team members in development of discharge plans.
  • Maintains ongoing communication with multidisciplinary team members regarding patient status and follow-up.
  • Attends rounds in major practice areas.
  • Documents consultation, recommendations, interventions and treatment plans.
  • Communicates needs for patient care conferences.
  • Refers to internal resources of hospital that can assist patients and families.
  • Refers basic community resources.
  • Assist with discharge planning for patient and family.
  • Provides referrals for ongoing assistance to patient/family.
  • Consults and intervenes in crisis situations.
  • Assists staff in de-escalating volatile situation.
  • Participate in critical incident debriefings.
  • Assist Clinical staff with locating Community Resources.
  • Assist with referrals to patient's family for ongoing assistance after leaving program.
  • Assist with facilitating a timely discharge of patients.
  • Reports all mandatory protective concerns such as Elder Abuse, Child Abuse and Domestic Violence ad required by law.
  • Protects patient privacy by maintaining confidentiality.
  • Keeps current with regulatory and legal requirements regarding patient privacy and confidentiality standards.
  • Identifies bio-ethical dilemmas and initiates appropriate response.
  • Assess discharge plan (may complete psychosocial assessments as appropriate per scope of practice and program). Identifies the key clinical issues such as grief and loss, adjustment to illness, psychiatric concerns with attention paid to the developmental milestones and life transitions that can impact functioning.
  • Practices appropriate boundary setting with clients.
  • Demonstrates sensitivity to cultural and ethnic diversity.
  • Effective at crisis intervention and problem solving techniques and is a resource to others in these areas.
  • Seeks resources when faced with unusual or new situations.
  • Evaluates patient/family response to interventions.
  • Revises care provided to patients/families.
  • Orient patients, initiate, and support admissions process per policy and or department guidelines.
  • Complete Utilization Review for admitting and ongoing treatment for patients. Document authorizations in electronic health record and per policy or department guideline.
  • Adequately orient new employees of rules and responsibilities.
  • Evaluate abilities and efficiency.
