Job Responsibilities: Participate in planning related to resident, family, and community interests. Participate in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary. Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay. Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available. Minimum Qualifications: Must possess a bachelor's or master's degree in social work or a bachelor's or master's degree in a human services field (i.e. sociology, special education, rehabilitation counseling and psychology.) One year of supervised social work experience in a healthcare setting working directly with individuals. Experience in long-term care, working with the senior population and dementia counseling. Must be licensed as a social worker in Michigan. Must lead by example and possess superior customer service skills and professionalism.
