Project FIND’s mission is to provide low- and moderate-income and homeless older adults with the services and support they need to enrich their lives and live independently. Today Project FIND operates four supportive housing residences that are home to about 7 00 people , four Older Adult Centers with over 3,000 members , a case management program that provides assistive services to over 300 older adults and a program that assists over 100 homeless individuals who are often street homeless . Our members and residents range from the healthy and active to the frail and homebound to the homeless. Please visit our website at: www.projectfind.org . Job Summary: The Social Worker will work with tenants who are older adults at the Woodstock Hotel location. The program provides case management services, information, referrals, supportive counseling, housing assistance , group work and crisis intervention for clients and their families. The Social Worker is responsible for conducting detailed assessments to determine client needs and eligibility for services and engaging with clients to develop goal-oriented service plans.
