About The Position

Project FIND’s mission is to provide low- and moderate-income and homeless older adults with the services and support they need to enrich their lives and live independently. Today Project FIND operates four supportive housing residences that are home to about 7 00 people , four Older Adult Centers with over 3,000 members , a case management program that provides assistive services to over 300 older adults and a program that assists over 100 homeless individuals who are often street homeless . Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these men and women, Project FIND is a critical resource providing housing, meals, support and programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living. Please visit our website at: www.projectfind.org . Job Summary: The Social Worker will work with tenants who are older adults at the Woodstock Hotel location. The program provides case management services, information, referrals, supportive counseling, housing assistance , group work and crisis intervention for clients and their families.  The Social Worker is responsible for conducting detailed assessments to determine client needs and eligibility for services and engaging with clients to develop goal-oriented service plans.

Requirements

  • Master’s degree in social work, human behavior, gerontology or related required
  • LMSW strongly preferred
  • A t least two years relevant experience in social services and knowledge of benefits and entitlements a plus.

  • Ability to provide accurate and timely documentation .
  • Time management skills to coordinate multiple responsibilities and services.
  • Ability to work independently and as part of a team, in both a home and office environment.
  • Maintain client and agency confidentiality.
  • Excellent interpersonal, public speaking, and written communication skills.
  • Must be able to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures.
  • Be skilled in conflict mediation/negotiation and have an assertive approach to problem solving.
  • Excellent oral and written communication skills.
  • Ability to utilize various computer programs, specifically Microsoft Word and Excel.
  • Be creative and flexible.

Nice To Haves

  • Experience working with older adult populations and families preferred.
  • Bilingual language Spanish fluency preferred.

Responsibilities

  • Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits, entitlements; conduct home , office and community visits to address unmet needs ; conduct interviews of shelter clients for tenancy .
  • Implement constructive short and long-term interventions through counseling education about counseling and physical health supports , long-term planning, and assistance with benefits and entitlements.
  • Provide orientation, support, and services necessary to assist tenants to remain permanently housed in a safe environment .
  • In collaboration with Property Management, assist with income affidavits , housing applications and recertifications and assist in collecting rent as needed.
  • Work closely with all departments within the agency, including property management, maintenance, housekeeping, and the Older Adult Center, to ensure comprehensive support for tenants. Additionally, be able to coordinate with collateral contacts, such as healthcare providers, home care agencies, family members and other formal and informal caregivers to promote tenants’ overall health, safety, and well-being.
  • Develop and create training events to educate clients on topics that can help support them in their daily lives (e.g., self-care, health and wellness, legal planning).
  • Lead one or more support groups, providing emotional support and psychoeducation about various issues related to aging in place, health and wellness , end- of life planning, mental health, substance use, cognitive well-being and self-care.
  • Provide timely documentation and maintenance of client files and progress notes.
  • After six months, assist with training and support of new staff on essential job functions , as requested .
  • Provide supervision, training, and support to paraprofessional staff, including students.
  • Perform other related tasks as requested or assigned.
