About The Position

In this role you will be: Assessing, planning, organizing, implementing, evaluating, and directing social service programs in accordance with existing state, federal, and local standards as well as facility-established policies and procedures. Assuring that the medically related emotional and social needs to the patients and families are met/maintained on an individual basis. Considering the special needs and behaviors of specific age groups. Able to understand the continuum of care in the health care system and possess the communication skills to interpret this system to patients, families, and other members of the health care team. Able to understand third-party payor systems as they relate to discharge planning. Utilizing basic computer skills and medical terminology to effectively communicate with professional staff. Participating in continued stay reviews with managed care payors. Ability to work in inpatient case management as needed.

Requirements

  • BS in health-related field
  • Graduate curriculum (leading to a master’s degree) in an accredited school of social work preferred or has the documented equivalent in education, training, and/or experience
  • Social work experience in acute care setting preferred
  • N/A
