About The Position

At St. Croix Hospice, the Social Worker is a vital part of the patient care team, providing compassionate emotional and practical support to patients and their families according to each individual's Plan of Care. This full-time position involves meeting with patients and their loved ones, collaborating with the clinical team, and assisting families in accessing appropriate community resources and services. The role is for a compassionate Social Worker who values meaningful patient connections and autonomy in their practice, guiding patients and their families through the end-of-life journey to ensure the highest quality of life with dignity, comfort, and respect.

Requirements

• Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education
• Current Social Work licensed in the state(s) of practice
• Ability to pass background, consumer report, and drug test
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

Responsibilities

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities
• Collaborate with the interdisciplinary team
• Maintain accurate documentation
• Support cultural and end of life needs
• Liaise with community agencies
• Ensure HIPAA compliance
