About The Position

Easterseals NY-Valhalla is looking for a temporary Social Worker for the summer session, which runs from 7/6-8/14. As a Social Worker, you will participate as a member of the admissions team and provide client counseling, complete social service assessments, and maintain treatment plans. You will also provide a range of education and planning for clients, families, and staff.

Requirements

  • Bachelor’s Degree in Social Work or related Human Service’s field
  • Two (2) years experience providing social work services to the respective community.
  • Demonstrate a genuine interest in working with the clients through their rehabilitative and psychosocial treatment process
  • Possess ability to function as an integral member of a multi-disciplinary rehabilitation team

Responsibilities

  • Complete social service assessments (to include providing client information on Advanced Directives) within required timeframe of admission as ordered by referring physician.
  • Participate in the development and maintenance of the master problem list and social work treatment plans, and identify appropriate short and long-term social service goals for clients.
  • Update the master treatment plan based on the changes to the master problem list and social work treatment plans.
  • Participate in developing and carrying out behavior management programs as indicated.
  • Provide social work assessment, counseling and referral as defined in the client’s treatment plan.
  • Assist client, family and team with discharge and long term planning.
  • Participate and provide recommendations in all relevant interdisciplinary team conferences.
  • Educate team members about specific psychosocial issues that may be impacting on client's treatment plan.
  • Establish rapport with families and caregiver support systems with client consent and facilitate family meetings as needed.
  • Support/educate clients to relevant social, economic and mental health concerns to maximize independent living in individual and group settings.
  • Facilitate appropriate psychotherapy and psycho-educational groups.
  • Carry on a positive working relationship with both inter- and intra-Agency sources.
  • Maintain professional ethics and adhere to the NASW code of ethics.
  • Maintain documentation as defined in Agency’s Policy Manual.
