Summary: The Social Worker is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families. Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high‑risk psychosocial factors of patients/families that impact health status and helps staff understand the influence of those factors upon the course of medical care. Collaborates with and provides social work consultation to other disciplines within the healthcare setting and the community.

  • Must display a high level of flexibility, adaptability and organizational skills.

  • Must communicate effectively and respectfully with patients, families, and all levels of staff and management.
  • Required to provide conscientious commitment to assigned work, work schedule, honesty, teamwork, and assisting others to bring about positive outcomes.
  • Must be able to recognize and manages risks, resolve issues and problems as soon as they arise, report and document adverse events and reportable conditions.
  • In all respects, verbally and behaviorally, must support Memorial Healthcare System’s mission and values.
  • Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.
  • Required to identify and intervene for suspected abuse including sensitivity to age specific concerns: physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; caretaker/institutional abuse for senior adults.
  • Must be able to manage crisis situations, demonstrating ability to quickly evaluate crisis situation and select appropriate intervention.
  • Must be able to document direct service in patient’s medical record after each significant encounter.
  • Must demonstrate proficiency in use of the electronic medical record.
  • Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.
  • One (1) year of social work experience in a health care setting.

Responsibilities

  • Responsible for outreach and coordination with other community resources and agencies, and community-level advocacy on behalf of patients and families.
  • Responds to requests for assistance with Social Determinants of Health and advocates for support in the community.
  • Refers to and collaborates with Memorial Healthcare System (MHS) SDOH Hub.
  • Assesses patient/family learning needs, styles and readiness.
  • Educates patients/families based on treatment plan, identifies barriers to care, diversity issues and learning styles.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Provides psychosocial support and interventions, particularly for individuals with chronic and/or complex medical and behavioral health conditions.
  • Provides assessment and intervention in domestic violence and child abuse situations.
  • Supports the department's initiatives, mission and goals and may take on additional responsibilities at the discretion of leadership.
  • Responsible for identification and referral for specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
  • Performs all aspects of psychosocial discharge planning for chronic conditions and/or long-term custodial care needs for inpatient discharge, community services, outpatient care and any other applicable setting.
  • Provides assistance with entitlements, medications, transportation, and advance directives.
  • Supports patients with mental health needs during the episode of care and discharge planning to both inpatient and outpatient mental health providers.
  • Develops comprehensive bio-psychosocial assessments responsive to age appropriate and cultural needs and concerns.
  • Employs a range of clinical interventions such as adjustment to illness counseling, crisis intervention, care coordination, and information and referral and safety planning.
  • Advocates on behalf of patients and families to gain access to services and resources.
