About The Position

The Master’s prepared social worker provides the professional social work component of the Case Management services. Case Management in Hospital/Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self determination.

Requirements

Master’s Degree of Social Work required. Relevant certification or progress toward relevant certification preferred.
Georgia Licensure Preferred
Minimum graduate level internship in medical setting. Prefer 3 years acute care experience in a social worker role.
The Master’s prepared Social Worker must be a highly motivated, self-directed individual with a demonstrated understanding of acute care case management issues. The individual will have excellent verbal and written communication skills, strong listening skills, critical thinking and analytical skills, ability to build positive working relationships with the interdisciplinary team, ability to communicate with multiple levels in the organization (e.g. managers, physicians, clinical and support), government agencies and post-acute providers. He or she must have the ability to obtain and interpret information in terms of the patients’ needs that are age appropriate, and the ability to advocate/negotiate patient/family assessed needs with Durable Medical Equipment / Home Health Care agencies, Nursing Facilities, and/or insurance companies. Must have the ability to teach and mentor social workers with a BS degree.
