Summary: The Social Worker will work in collaboration with members of the interdisciplinary team to enhance the cost-effective utilization of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings.

Requirements

• Master’s degree in Social Work
• Active and unrestricted Missouri LMSW or LCSW
• Minimum of 2 years hospital experience
• Must have in depth knowledge of system and community resources and referral agencies
• Ability to communicate effectively with other healthcare agencies and practitioners
• Team, detail and goal oriented
• Positive, professional and empathetic demeanor

Responsibilities

• Provides assistance to patients, families, and others with a wide variety of needs to include, but not limited to, discharge planning, financial concerns, and emotional support as related to the current acute illness.
• Provides supportive counseling to patients regarding adjustments to illness as well as other biopsychosocial needs as indicated.
• Communicates with external agencies to provide referral information for a smooth transition from hospital or the post-hospital setting.
