About The Position

Social Workers in this position work on a team with Nurses and Community Health Workers to case manage Medicaid eligible pregnant women and infants up to age one and their families. The social worker offers support, education, systems navigation guidance, linkage to community resources, and encouragement to nurture health and wellness throughout pregnancy. The social worker (as a member of the home visiting RN/BSW dyad) will visit the expectant mother in their home and for up to 12 months after the child is born. We are seeking a compassionate and skilled Social Worker to join our team in delivering patient-centered care across home and clinical settings. This role requires independent judgment in planning, coordinating, and evaluating care while working within established policies and procedures. The Social Worker will conduct assessments, develop care plans, and support patients in overcoming barriers to care.

Requirements

  • Education: Required: Bachelor’s Degree in Social Work (BSW)
  • Licensure: Required: Current Michigan licensure as a Licensed Social Worker
  • Experience: Required: Minimum 1 year of social work experience with families or equivalent education/experience
  • Skills & Abilities: Strong critical thinking and prioritization skills
  • Excellent verbal and written communication
  • Ability to manage crises and maintain professional composure
  • Knowledge of psychosocial interventions and community resources

Nice To Haves

  • Preferred: 2+ years in a healthcare setting; experience in community health, case management, and patient advocacy
  • Preferred: Fluency in Spanish

Responsibilities

  • Provide home visiting services, interviews, and assessments for physical and emotional health, economic condition, safety, and family supports
  • Assess social and emotional factors related to illness, need for care, and adjustment to prenatal/postpartum progress
  • Complete psycho-social assessments of client needs, strengths, and weaknesses; monitor progress toward positive pregnancy outcomes
  • Develop care plans based on input from client, staff, and family; monitor implementation and follow-up
  • Provide comprehensive case management and coordinate services through a multidisciplinary approach
  • Offer medical crisis counseling and problem-solving support
  • Coordinate support services in areas such as housing, mental health, transportation, and other basic needs
  • Network and maintain relationships with community agencies; advocate for client needs
  • Document all client contact and maintain accurate case records
  • Make referrals to Early On as needed
  • Participate in MIHP weekly meetings, Home Visiting Conference, and Quality Assurance Initiatives
