Requirements
- Current state licensure, or eligibility for licensure as required
- Strong skills in psychosocial assessment, counseling, and care coordination.
- Ability to support patients and families through serious illness, end-of-life care, and grief with compassion and professionalism
- Excellent communication, documentation, and time management skills.
Responsibilities
- Assess patient and family emotional, social, financial, and environmental needs (coping, safety, caregiver strain, resources, advance care planning readiness).
- Care coordination & resource navigation - connecting patients/families with community resources and coordinating with the interdisciplinary team.
- Participate in care plan development and case conferences.
- Serve as liaison between patients or families/caregivers and community agencies.
