Day Shift - 7.5 Hours (United States of America) Job Summary: The Social Worker will provide clinical and case management care to patients with intellectual and or developmental disabilities across the life span in an outpatient setting. Documentation must be in a timely and quality manner. Conduct individual, group, and family therapy. Assess, identify, and provide services for patients/families; address psychosocial concerns as well as post hospital care, including crisis intervention, advocacy and community referrals. Requirements

• Master's degree required.
• NYS Registration/Certification as a Social Worker (LMSW/LCSW) is required.
• 2 yrs post graduate work preferred.
• Prior experience in a hospital or community health agency is preferred.
• Experience with assigned patient population is required.
• Additional language fluency and computer literacy preferred.
• Experience treating concurring substance abuse and mental illness preferred.
• Knowledge of medical/health related setting and/or specialty area experience preferred.

Nice To Haves

• Membership in the NASW Academy is a plus.
