Adherence to post discharge plan(s) of care and the holistic health and functioning of the patient/family. Serves as a member of the interdisciplinary health team. Collaborates with departmental/organizational support services. Assists in the development, maintenance and evaluation of the Prisma Health Medical Management Social Work Services and performance improvements. Requirements

• Education - Master's degree in Social Work
• Experience - Two (2) years of experience in health-related area
• Licensed Master of Social Work (LMSW) or Licensed Independent Social Work (LISW).
• BLS Current Heart saver certification
• Psychosocial intervention skills.
• Analytical and problem-solving abilities.
• Telephone communication skills.
• Ability to work within interdisciplinary setting.

Nice To Haves

• ACM preferred.

Responsibilities

• Provides psychosocial assessment and interventions.
• Coordination and collaboration with ancillary services, providers, and agencies.
• Implementation/coordination of identified discharge planning/placement needs.
• Works closely with others in an effort to optimize quality and minimize cost.
• Provides post-discharge follow-up when deemed appropriate.
• Assists in development/maintenance and evaluation of clinical data.
• Interprets data and makes recommendations based on results.
• Utilization of paper and computerized charting.
• Supports product line/medical staff quality improvement initiatives.
• Rotational weekend and weeknight calls.
• Performs other duties as assigned.
