The Social Worker assists with the acclimation of admission for each resident as well as discharge planning upon leaving the facility. They provide direct resident and family support and acts as a resident advocate while working collaboratively with our nursing and management team to provide the best experience possible. Requirements
- High School Diploma or equivalent
- Within the last five years, must have one year of supervised social work experience (preferably in a healthcare setting)
- Must have valid Virginia driver's license
Responsibilities
- Assists with the acclimation of admission for each resident
- Discharge planning upon leaving the facility
- Provide direct resident and family support
- Acts as a resident advocate
- Working collaboratively with our nursing and management team
