About The Position

Social Worker - Health Care Center Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a compassionate professional to become a part of our social services team to help provide our residents with the highest quality of life. In our Health Care Center, you will find our outstanding nursing team assisting residents in four distinct neighborhoods - specializing in long-term care, memory care and short-term rehab. Our team members work together to create a more active and purposeful life for our residents, so they can lead healthier and more fulfilled lives. Job Description: As a Social Worker, your scope of responsibilities will include but is not limited to: Contributing to the development, implementation, and ongoing evaluation of Social Services programs designed to meet and assist residents in attaining or maintaining their highest practicable well-being. Completing comprehensive psychosocial assessments and creating, developing and implementing interventions in order to improve and maintain the resident's quality of life. Assisting with the admission process by conducting interviews, assessments and documentation in compliance with state/federal regulations. Completing required documentation and record keeping including social history, psychosocial assessment, progress notes, discharge plans and gradual dose reduction in compliance with the state/federal regulations. Collecting resident information to help other health professionals understand the emotional, social, spiritual and environmental factors that may impact a resident's well-being. Acting as the resident's advocate and assisting residents and families with emotional, psychological and spiritual issues associated with end of life care. Requirements

  • Must hold a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling or psychology.
  • At least one year of social work experience in a health care setting required.
  • Must be able to communicate clearly verbally and in writing, maintain positive interactions with residents, team members, families and visitors, understand the aging process and have working knowledge of relevant medical/legal issues that impact resident care.
  • Must be able to analyze problems and participate in identifying and implementing solutions as well as responding to crisis situations.

Nice To Haves

  • Previous experience in a long-term care setting preferred.
