About The Position
Harlem Center for Nursing and Rehabilitation is looking for a Social Worker to join the team at our Skilled Nursing Facility in Harlem, NY! Who We Are The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. We pride ourselves in providing a high standard of care for patients of all ages and conditions, while upholding an environment of kindness, dignity, and respect for employees and residents alike. Requirements
- Bachelor’s degree in Social Work Preferred
- Previous experience in a supervisory capacity in a skilled nursing facility
Responsibilities
- Ensure that the medical, social, and emotional needs of each resident are provided for
- Work together with other staff to identify potential difficulties for residents
- Serve as a resident advocate
- Provide crisis intervention, counseling, or other services to residents and families
- Assist new residents and families in the initial adjustment phase
- Work with residents and families on planning discharges
- Act as a liaison to Federal, State, and local agencies providing service to the resident community
- Maintain a productive working relationship with other department supervisors
- Coordinate social services to assure the daily social services can be adequately provided
