Scheduled Weekly Hours: 36 The Social Services team is looking for a WEO (weekend only) Social Worker to join the team in La Crosse, WI! The WEO Social Worker covers all inpatient medical surgical units (IPR, ICU, Gen Med, Ortho, Neuro, Med Spec, all maternal child units, Cardio/Pulm, Heritage Unit, Surgical Digestive and Medical Oncology). What you will work: .9 FTE = 72 Hours every 2 weeks Friday, Saturday, Sunday 6am-6pm 12 hours shifts What you will do: Provide a variety of services to our acute care patients including discharge planning, care coordination, care navigation, resource connections, supportive listening, ACP completion and long-term care planning Assess complex biopsychosocial needs and provides interventions to support the individualized treatment plan. Act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals. Requirements

  • WI Social Worker license
