About The Position

The Hospice Social Worker at Bristol Hospice plays a vital role in providing emotional and practical support to patients and their families during end-of-life care. This position focuses on assessing psychosocial needs, offering counseling, and facilitating communication between patients, families, and healthcare teams to ensure dignity and comfort in challenging times.

Requirements

  • Must have a master's degree in social work from an accredited university.
  • Minimum of two (2) years of documented supervised experience in health care, hospice experience preferred.
  • Understanding of hospice philosophy and issues of death/dying.
  • Certified Hospice and Palliative Social Worker is desirable.
  • Flexibility in work hours and ability to travel throughout the assigned service area.
  • Willingness to maintain knowledge regarding information systems and applicable software programs.

  • Experience in hospice care or palliative care settings.
  • Knowledge of community resources and agencies related to hospice care.

Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
  • Provide assessments in the patient's residence and assist when safety is a concern.
  • Carry out social evaluations, including family dynamics and caregiver abilities, and plan interventions based on findings.
  • Counsel patients and families/caregivers regarding stress and coping difficulties, providing crisis intervention when necessary.
  • Educate the interdisciplinary group on special needs related to the culture of the patient and family.
  • Maintain clinical records on all patients referred to social work.
  • Educate patients and families on advanced directives and assist in their preparation.
  • Provide information and referral services for practical and environmental needs.
  • Serve as a liaison between patients, families, and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Participate in the development of individualized care plans and attend interdisciplinary meetings.
  • Assist team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Participate in quality assessment performance improvement activities.
  • Assist families and patients in planning for funeral arrangements and decision responsibilities.
  • Perform other duties as delegated by the Clinical Supervisor.
