About The Position
The Social Worker in a Patient Aligned Care Team provides clinical and psychosocial interventions in a multi-disciplinary team to Veterans. The Incumbent is a professional social worker assigned to the Primary Care Clinic. The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans.
