About The Position

Requirements

  • Bachelor's or Master's degree in social work from an accredited program.
  • Previous social work experience, preferably with geriatric patients.
  • Knowledge of the case management process and funding resources such as Medicare and Medicaid.
  • Experience in dealing with people in crises.
  • Strong communication skills, both verbal and written.
  • Ability to work cooperatively with other professionals and exercise independent judgment.

Responsibilities

  • Coordinate admissions to the center in the absence of the Admissions Coordinator.
  • Conduct needs assessments for patients seeking admission and consider alternatives if the center cannot meet their needs.
  • Share information about funding resources with patients and families.
  • Assist individual patients and families in adjusting to center life and dealing with crises related to illness, disability, and death.
  • Serve as an advocate for patients and their families in expressing, defining, and resolving grievances.
  • Coordinate discharge planning functions for patients.
  • Participate in developing patient care plans in collaboration with other disciplines.
  • Engage in in-service training programs to enhance skills and knowledge.
  • Evaluate the quality of social services as part of the center's quality improvement program.
  • Develop and maintain contacts with community agencies for resource sharing.
  • Handle marketing activities/functions as assigned by the SWS Department Head.
