About The Position

Requirements

• Social worker degree
• A social worker license required
• Previous LTC/SNF experience preferred
• Ability to work independently or as part of a group
• Computer knowledge

Responsibilities

• Conduct psychosocial assessments of residents to identify their needs, strengths, and preferences.
• Develop individualized care plans in collaboration with the interdisciplinary team to address the psychosocial needs of residents.
• Provide counseling and emotional support to residents and their families.
• Assist residents in navigating and accessing community resources and services.
• Coordinate and participate in family meetings to discuss resident care plans and address any concerns.
• Collaborate with the healthcare team to ensure holistic and integrated care for residents.
• Advocate for residents' rights and ensure they receive respectful and dignified care.
• Facilitate group activities and programs to enhance the social well-being of residents.
• Maintain accurate and up-to-date documentation of all social work interventions and assessments.
