About The Position

As a vital member of the interdisciplinary care team, the Social Worker I plays a key role in delivering compassionate, patient-centered psychosocial support. In close partnership with patients, families, physicians, case managers, and other healthcare professionals, this role provides direct social work services including comprehensive assessments, behavioral interventions, goal setting, and care plan development. You'll be at the forefront of care coordination—breaking down psychosocial barriers, crafting discharge strategies, connecting patients to critical resources, and guiding them through complex transitions. From crisis intervention to patient advocacy, you'll be a key voice in optimizing outcomes and ensuring every patient receives holistic, high-quality support.

Requirements

  • Master's degree in Social Work (MSW) required.
  • Licensed Master Social Worker (LMSW) in the state of Maryland.
  • Ongoing professional development and training as required.
  • Up to one year of experience in a healthcare-related setting preferred.
  • Relevant field placement may be considered as qualifying experience.

Responsibilities

  • Proactively screen assigned patients for social work needs, initiating timely consults.
  • Conduct in-depth psychosocial assessments and develop individualized care plans.
  • Manage a full caseload, providing expert psychosocial support and transition planning.
  • Collaborate closely with the multidisciplinary care team to ensure seamless care coordination and goal achievement.
  • Document findings, plans, and interventions in compliance with regulatory and departmental standards.
  • Maintain a strong network of internal and external referral resources, continuously seeking out innovative community partnerships.
  • Assist in sensitive matters such as psychiatric referrals, capacity assessments, abuse/neglect evaluations, guardianships, end-of-life care, and more.
  • Facilitate and lead family meetings and participate in interdisciplinary rounds as needed.
  • Contribute to quality improvement efforts and represent social work perspectives on internal and external committees.
  • Provide on-call coverage and occasional home visits when necessary.
  • Engage in regular supervision in compliance with COMAR regulations.
  • Perform other duties as assigned, always contributing to the mission of holistic, patient-focused care.
