About The Position
ACCELerate Piscataway is seeking a dedicated Social Worker to join our team. In this role, you will be responsible for providing essential services to our patients and residents, ensuring their physical, mental, and psychosocial well-being. Your work will involve collaborating with an interdisciplinary team to develop and implement comprehensive care plans tailored to each individual's needs. You will engage with patients, their families, and staff to identify strengths and areas of need, facilitating access to necessary services and resources. As a Social Worker, you will play a crucial role in discharge planning, which includes educating patients and families about the process, documenting necessary information, and making referrals to appropriate agencies. You will also be responsible for holding meetings with patients and their families to provide support and improve their quality of life. Establishing and maintaining relationships with community-based agencies will be vital to ensure our residents receive the best possible care and support.
