About The Position
The Social Worker at Housecall Providers is responsible for delivering comprehensive social work services to patients, particularly those who are homebound or suffering from advanced illnesses. This role involves conducting psychosocial evaluations, providing ongoing counseling, and coordinating referrals and transitions of care. The Social Worker will work closely with patients in various settings, including their homes, assisted living facilities, hospitals, and clinics, to ensure they receive the necessary support and care tailored to their individual needs. In this position, the Social Worker will conduct thorough psychosocial assessments and make appropriate referrals to community resources. They will educate patients and their families about available benefits and entitlements, and collaborate with clinical staff to address any issues that arise. The role also includes providing counseling and support to patients and their families, helping them develop coping strategies for managing declining health and emotional needs. The Social Worker will actively participate in interdisciplinary care teams, assisting in the establishment and implementation of psychosocial care plans for patients with serious and complex illnesses. They will also facilitate advance care planning discussions, ensuring that patients' goals of care are communicated and respected. This position requires a compassionate approach, as the Social Worker will advocate for patients and families, assist with crisis intervention, and provide education on mental health issues and community resources.
