About The Position
Bethany Home Sioux Falls is seeking a dedicated and experienced Social Worker to join our team in a full-time capacity. In this key role, you will work closely with our residents to identify and address their psychosocial, mental, and emotional needs. Your primary responsibility will be to help provide, develop, and facilitate access to services that meet these needs, ensuring that our residents achieve the highest possible level of physical, mental, and psychosocial well-being. You will play a critical role in fostering an environment that promotes individuality, independence, and dignity for our residents. As a Social Worker, you will be involved in reviewing facility policies and procedures as part of the interdisciplinary team to ensure compliance with state and federal regulations. You will develop, maintain, and utilize a current list of community resources beneficial to residents and their families or significant others. It is essential that all social service documentation meets government requirements, and you will collaborate with the interdisciplinary team and administration to protect resident rights and promote psychological well-being. Additionally, you will be responsible for preventing and addressing resident abuse as mandated by law and professional licensure. Providing support, information, and organization to residents, families, significant others, and staff will be crucial in encouraging proactive self-advocacy and improving the quality of life and care within the nursing home and the community. You will also facilitate discharge planning, ensuring a smooth transition for residents leaving the facility.
