About The Position
The Social Work Care Manager at Intermountain Healthcare plays a crucial role in enhancing the health outcomes of patients with chronic conditions or complex needs. This position requires collaboration with physicians and other healthcare team members to provide comprehensive care management. The Social Work Care Manager is responsible for educating patients and their caregivers, helping them navigate their healthcare needs effectively. This role involves facilitating communication among various stakeholders, coordinating services, and addressing barriers to care, all while ensuring a balance between clinical quality and cost management. In this position, the Social Work Care Manager will identify patient populations through specific screening criteria, including psychosocial assessments and medical record reviews. They will gather relevant data by interviewing patients, families, and significant others, ensuring that a plan of care is developed and maintained in accordance with established standards. The role also requires coordination of communication among patients, caregivers, and healthcare team members, as well as external parties such as referral physicians and community organizations. The Social Work Care Manager will actively intervene to resolve barriers to service across the care continuum and maintain up-to-date knowledge of community resources. They may also perform cost-benefit analyses and coordinate authorizations for appropriate levels of care. Additionally, the role may involve diagnosing mental, emotional, and behavioral disorders using DSM-5 guidelines and providing various social work interventions as needed. The position requires adherence to the Mental Health Practice Act and collaboration with external agencies when abuse, neglect, or exploitation is suspected or confirmed.
