About The Position

The Social Worker at the University of Rochester is responsible for providing clinical social work services to patients and their families within a medical setting. Requirements

  • Supervisory experience is preferred.
  • Familiarity with medical social work and community resources is preferred.

Responsibilities

  • Provides professional social work services to patients and their families.
  • Collaborates with hospital staff and community social services resources regarding patients' psycho-social problems.
  • Participates in formulating procedures and policies of the Division of Social Service.
  • Participates in educational and research programs, including the orientation and training of new staff and students.
  • Coordinates discharge planning for patients at psychosocial risk in collaboration with healthcare teams.
  • Facilitates referrals to appropriate community agencies and hospital resources.
  • Conducts psychosocial assessments and provides social work interventions, including crisis intervention and grief counseling.
  • Handles risk management issues in consultation with supervisors.
  • Documents social work activity in the electronic medical record.
  • Provides social work coverage to other clinical units during co-worker absences.
  • Completes required in-service training and ongoing professional development activities.
  • Attends Social Work Division meetings and complex case discussions.
  • Supervises social work students and provides peer support to new staff.
