The Social Worker at Livingston Place is responsible for overseeing and managing the social services needs of the residents within our community. Regular reviews and updates of care plans will be necessary to ensure they align with the evolving needs of the residents. Advocacy and support are key components of this role. The Social Worker will provide emotional support to residents and their families during difficult situations, assist with financial qualifications for government-assisted programs, and offer counseling and guidance to address social and emotional challenges. Collaboration with the interdisciplinary team is essential to address complex social issues affecting residents, and staying informed about changes in legislation and regulations related to social services and HCBS long-term care settings is crucial. Effective communication is vital in this position. The Social Worker will collaborate with healthcare professionals, communicate effectively with residents and families, and participate in interdisciplinary team meetings to enhance overall resident care. This role requires a strong knowledge of regulatory requirements and compliance in the long-term care industry, as well as excellent communication skills, problem-solving abilities, and the capacity to handle multiple demands under pressure.
