About The Position
Swank Center for Memory Care and Geriatric Consultation is looking to hire a Social Worker. Located at 501 West 14th Street, Wilmington, DE 19801, with additional locations in Smyrna and Rehoboth, Delaware, the Swank Center has been selected as a GUIDE model by the Centers for Medicare & Medicaid Services (CMS), enhancing its ability to provide comprehensive dementia treatment plans and necessary resources to caregivers. As the Social Worker, you will join the Guide team in supporting the Community Educator, conducting home visits, and assisting patients and families in identifying social, emotional, and environmental needs. This role aims to support an innovative healthcare model.
