About The Position

We are seeking a qualified Social Worker to support residents, patients, and families within a healthcare or long-term care setting. The Social Worker will provide psychosocial assessments, care planning support, and advocacy services while collaborating with interdisciplinary teams to ensure quality outcomes and continuity of care.

Requirements

• Degree in Social Work (BSW or MSW, per state and facility requirements)
• Active Social Worker license or certification as required by state regulations
• Strong communication, documentation, and organizational skills
• Ability to work independently and as part of an interdisciplinary team

Nice To Haves

• Prior experience in healthcare, skilled nursing, or long-term care preferred

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans
• Provide counseling and emotional support to patients and their families
• Assist with discharge planning and coordination of community resources
• Advocate for patient rights, needs, and preferences
• Collaborate with nursing, therapy, and medical staff to support patient goals
• Document assessments, interventions, and progress notes accurately and timely
• Participate in care conferences and interdisciplinary team meetings
• Ensure compliance with state, federal, and facility regulations
• Support patients with adjustment, transition, and end-of-life concerns
