About The Position

The position involves assisting patients with behavioral health needs as an extender for primary care providers at Strawberry Wilderness Community Clinic (SWCC). The role focuses on providing intervention services, assessments, referrals, and short-term therapy, while also developing behavioral change plans and protocols for target populations. Requirements

  • Bachelor's Degree in Social Work supervised by LCSW.
  • Basic Life Support (BLS) certified by American Heart Association (AHA), must obtain within 60 days of hire.
  • Valid Permit or Driver's License.

  • Working experience in hospice or medical, critical access hospital setting.
  • Master's Degree in Social Work.
  • Licensed Clinical Social Worker.
  • Special Education in Grief.

Responsibilities

  • Provide an active presence to the SWCC Family Care clinical teams and accept warm hand-offs for assessment and treatment interventions.
  • Coordinate and collaborate with primary care providers to improve health outcomes.
  • Provide therapeutic services to patients and their families.
  • Facilitate referrals and coordination to other services as needed or directed by primary care providers.
  • Develop and maintain a professional relationship with local mental health agencies to enhance patient referrals.
  • Work to meet patient needs effectively and resolve barriers through follow-up and collaboration with community service providers.
  • Maintain accurate and timely records of activities, case management notes, and services provided to each client.
  • Submit encounter and billing data in a timely fashion.
  • Work with the primary care team to manage patients with chronic emotional or health problems.
  • Assist in the detection of at-risk patients and develop plans to prevent further deterioration.
  • Assist in preventing relapse or morbidity in recurring conditions.
  • Teach patients, families, and staff about care, prevention, and treatment enhancement techniques under provider direction.
  • Provide occasional consultation and case management to patients at Blue Mountain Hospital.
  • Participate in transition of care planning for patients with mental or behavioral health needs.
  • Assist in developing policies and procedures for integrated mental and behavioral health services.
  • Advise the Clinic Manager on licensure and policy needs for regulatory compliance.
  • Model and assure professionalism and excellent customer service standards.
