About The Position
The position is responsible for providing social work services to patients, families, and individuals, including psychosocial assessment, screening, determination of needs evaluation, appropriate interventions and follow up, and discharge planning. The role involves implementing interventions based on assessments, participating in a multi-disciplinary health care team, advocating for patient involvement in care plans, and maintaining knowledge of community resources and regulations impacting health care delivery. The social worker will also provide support for bereavement, ethical issues, and connect patients to appropriate agencies for social matters. The position requires effective communication with patients, families, and other health care team members, as well as documentation of activities according to departmental standards. The social worker will manage patient care to optimize length of stay and decrease unnecessary readmissions, and will also participate in the orientation of new staff and education of social work students.
