As a Social Worker for Sports Cardiology and the WISER Program (Women’s Integrated Sports, Exercise, and Research), you will be part of an innovative multidisciplinary care model delivering comprehensive support to adult women athletes of all levels. This role integrates behavioral health services within the care team and extends to patients of the Heart, Vascular & Thoracic Institute, providing mental health support following cardiac events, procedures, and new cardiac diagnoses. You will apply your expertise in crisis intervention, resource navigation, mediation, counseling, and patient empowerment to support individuals and families from diverse backgrounds and with varying needs. By helping patients navigate cardiac and athletic health challenges, you will improve mental well-being, recovery, and overall quality of care. A caregiver in this role works days from 8:00 a.m. – 4:30 p.m.

Requirements

  • Master’s Degree in Social Work from a CSWE accredited program
  • Current state licensure as a Licensed Social Worker (LSW)
  • State licensure as a Licensed Independent Social Worker (LISW) within five years of hire
  • Two years of experience in a hospital or healthcare setting
  • Manual dexterity to operate office equipment.
  • May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
  • Must have normal or correction vision
  • Must be able to clearly communicate verbally by phone.
  • Follows Standard Precautions using personal protective equipment as required.

Responsibilities

  • Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
  • Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings).
  • Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
  • Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
  • Maintain a network of resources for consultation and connect patients with community resources.
  • Supervise Social Work Interns and assist with training new caregivers.
  • Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
