About The Position

The Social Worker/Senior position at UTHealth Houston involves providing professional social work services within the General Medicine Clinic. Requirements

  • Licensure as a Master Social Worker (LMSW) by the Texas Board of Social Work.
  • Master's Degree in Social Work or related field.
  • One year of social work experience required.
  • Medical social work experience preferred.

Responsibilities

  • Conduct comprehensive assessments to interpret patients' social, emotional, environmental, and financial needs.
  • Plan treatment courses or refer cases to appropriate professionals or agencies to ensure patients receive necessary treatment.
  • Obtain data from patients and relatives to assist in developing significant teaching programs and social services.
  • Collaborate with physicians and multi-disciplinary team members to coordinate care and enhance patient experience.
  • Develop patient goals, monitor progress, address barriers, and evaluate outcomes of individualized care plans.
  • Conduct individual and group counseling sessions.
  • Prepare detailed reports and may video-tape sessions for review with coworkers or faculty members.
  • Solicit assistance from religious, social, and welfare agencies in patient treatment.
  • Educate healthcare providers on community resources and psychosocial factors affecting care.
  • Utilize crisis intervention, conflict resolution, and mediation to address patient/family concerns.
  • Respond to emergency situations.
  • Perform other duties as assigned.
