About The Position

Requirements

• Licensure as a Master Social Worker (LMSW) by the Texas Board of Social Work.
• Master's Degree in Social Work or related field.
• One year of social work experience required.
• Medical social work experience is preferred.

Responsibilities

• Conduct comprehensive assessments to interpret patients' social, emotional, environmental, and financial needs.
• Plan courses of treatment or refer cases to appropriate professionals, institutions, or agencies.
• Obtain data from patients and/or relatives to assist in developing significant teaching endeavors, programs, and social services.
• Assess patients and/or relatives to understand individual problems and needs, collaborating with the multi-disciplinary team to coordinate care.
• Develop patient goals, monitor progress, address barriers to goal achievement, and evaluate outcomes of individualized care plans.
• Conduct individual and/or group counseling sessions.
• Prepare detailed reports and may video-tape sessions with patients for review.
• Solicit assistance from religious, social, and welfare agencies in patient treatment.
• Educate healthcare providers on community resources and psychosocial factors affecting care.
• Utilize crisis intervention, conflict resolution, and mediation to address patient/family concerns.
• Respond to emergency situations.
• Perform other duties as assigned.
