The position supports patient and family needs through an advocacy role, aiming to effect change in behavior and provide biopsychosocial intervention as necessary. The role involves assisting the Case Coordinator and Healthcare Team, providing crisis intervention, and linking complex or long-term placements to appropriate community resources. The individual will work directly with staff to facilitate the achievement of quality and cost-effective patient outcomes in Rehabilitation.

  • Support patient and family needs through advocacy.
  • Provide biopsychosocial intervention as necessary.
  • Assist the Case Coordinator and Healthcare Team.
  • Conduct crisis intervention.
  • Link complex or long-term placements to community resources.
  • Facilitate achievement of quality and cost-effective patient outcomes in Rehab.
  • Master’s Degree in Social Work.
  • Current Indiana LSW or LCSW license.
  • 2-3 years of experience in hospital or healthcare social work functions such as counseling, psycho-social assessment, and crisis intervention.
  • Experience using a computer related to necessary applications.
  • Ability to work flexible hours.
