About The Position

Serves as the HUD-VASH Program Manager for the San Francisco VA. The program serves 9 counties and 7 CBOCs with over 2700 vouchers. Responsible for managing the day-to-day operations, provides programmatic direction and coordination to all staff assigned to the HUD-VASH Program, including social workers, clinical support staff, program supports, and associated providers. Responsible for developing and overseeing systems for admission and ongoing case management services in the HUD-VASH Program Duties and responsibilities include but are not limited to: To responsible for program coordination and administration across multiple sites including consultation, negotiation and monitoring in the SFVA catchment areas and PHAs in each of those areas. The role of the Program Manager serves as the VA SFMC primary point of contact for the National HUD-VASH Program, Office of Mental Health Services, VACO. The incumbent can translate management goals and objectives into well-coordinated and controlled work operations as well as analyzing organization and operational problems and develop and implement solutions that result in sound operation of the program. This is accomplished through data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements. To have a full understanding of the Veteran population, structure of the program, social work service, SFVA, VISN and VACO and understands the impact upon program operations. To develop policies, procedures and practice guidelines for the program and in participating in national program development. The incumbent ensures that the HUD-VASH Program operates in accordance with Social Work Service, Medical Center and VACO policy related to clinical care, documentation, safety, and information security. Similarly, the incumbent ensures that the operations of the HUD-VASH Program meet accreditation standards from The Joint Commission and CARF. To collaborate with internal and external partners and with VHA partners, both nationally and locally, to develop more effective and efficient ways of delivering healthcare services, further program goals and enhance patient centered care. They develop working relationships and agreements with other organizations, attend appropriate meetings in the community. To serve as a liaison between the HUD-VASH Program and the Public Housing Authorities (PHA's) responsible for the Section 8 Housing Choice vouchers Veterans receive in the HUD-VASH Program. They coordinate with the designated PHA regarding housing voucher application and award process for each veteran. The incumbent is responsible for reviewing the agreements between Section 8 landlords, Public Housing Authorities (PHAs), and the VA to establish appropriate referral sites for placements. The incumbent works with VA Medical Center multi-disciplinary teams as needed and community agencies to strive for to HUD-VASH Program veterans. In this liaison role, the incumbent meets frequently with PHA leadership to address challenges in coordination and planning, and to ensure that PHA's function according to HUD-VASH policy. The incumbent educates PHA's regarding VA performance measures and necessary time frames. To establish, implement, and maintain referral and screening procedures for potential HUD-VASH Veteran participants that meet national policy. Referral, screening, and admission criteria recognizes program eligibility, prioritization of subsets of the homeless veteran population, requirements of the PHAs, and other policy, regulation, or law pertaining to program administration. The incumbent develops the screening procedures to involve a multidisciplinary team approach and is responsible to ensure criteria and procedures are maintained. To create and maintains an effective and secure means of collecting data related to screening, referral, admission, ongoing case management, discharge, and clinical outcomes. The incumbent is responsible for creating and maintaining an effective and clinically meaningful process of program evaluation and prepares reports for service, medical center, VISN, and VACO use. The incumbent is responsible for ongoing data monitoring and reporting to VA Central Office. To perform accountability functions: 1) completes statistical, administrative, staffing studies and all other reports in accordance with Social Work Services, San Francisco Health Care System and VHA Program Offices policies. To provide and assures a full range of supervisory duties which include responsibility for assignment of work to be performed, performance evaluations, staff selection, training and recommendations of awards, advancements and if needed, disciplinary actions. They programmatic direction to all staff assigned to the HUD-VASH Program, including case managers, program support, and associated providers. The incumbent assists in the development of administrative and personnel documents such as position descriptions, functional statements, trainings, and performance appraisals. To work directly with Social Work Service administrative staff and management to ensure effective systems are in place related to personnel, fiscal, acquisition and materiel management, contracting, and staff vehicles.
