About The Position

Incumbent serves as a program coordinator at the GS-12 grade and is administratively responsible for clinical program oversight, monitoring, and administration providing treatment to patients in the VA Grant and Per Diem program. Serves as primary contact with the contracted grant and per diem housing provider to ensure that patient care is coordinated with VA Medical Centers, community health and agencies that serve homeless populations. Major Duties of the Grant and Per Diem Liaison Program Coordinator Include: Patient Care: Independently Performs clinical and specialized clinical social work functions Independently Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Coordinates community-based services, including information and referrals as eligible Identifies high-risk patients and provides case management services. Incumbent provides crisis intervention services, Establishes and maintains effective therapeutic relationships with veterans and their families. Maintains complete, accurate, timely and confidential medical record documentation of patient information according to policies/regulations. Performs other related duties as assigned. Coordination/Administrative: Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Performs a wide range of services and provides technical assistance to the medical center, State, local, and private agencies, and universities. Performs work related to the review, monitoring, analysis, planning and assessment of the Grant and Per Diem program. Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer. Establishes and maintains effective channels of communication with numerous and varied programs. Participates on Medical Center, VISN and/or National councils, committees and workgroups as designated. Provides consultation and guidance to colleagues, role model effective social work practice skills, teaches or provides orientation to less experienced social workers, develops innovations in practice interventions, and provides clinical supervision for social work licensure or certification.
