About The Position

Assist patients who have complex psychosocial needs, and requiring assistance with eligibility determination for social programs and funding sources as well as qualification for community assistance from a variety of special funds and agencies. Offer short-term counseling and crisis intervention to patients and families with psychosocial needs, coordinate and facilitate the development of a safe discharge plan of care for high-risk patient populations.

Requirements

• Graduate from an accredited Social Work Program with a Bachelor's in Social Work.
• Clinical rotation experience can be considered in lieu of experience if applicable.

• Familiarity with community resources.
• Understanding of regulatory and insurance reimbursement issues.
