About The Position

Requirements

• Master's degree in social work from an accredited university.
• Two years of social services experience in a hospital setting.
• Licensed Medical Social Worker (LMSW) certification.

• Licensed Clinical Social Worker (LCSW) certification.

Responsibilities

• Provide compassionate clinical social work services, including psychosocial assessments and treatment.
• Collaborate with physicians and healthcare teams to facilitate quality patient care.
• Assist patients and families in addressing social, financial, and psychological issues related to health conditions.
• Communicate effectively with healthcare team members and report pertinent patient data.
• Educate staff on patient populations and department processes.
• Lead case management activities and identify at-risk patients.
• Conduct comprehensive assessments and develop psychosocial treatment plans.
• Manage conflict and provide crisis intervention as needed.
• Educate patients and families on discharge planning and resources available post-discharge.
• Document patient assessments and treatment plans accurately.
