About The Position

Seeking Medical Social Worker! PRN (work as needed), shifts and days vary Baptist Health Floyd, Care Coordination Southern Indiana/Louisville, Kentucky area The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting. This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization. The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge. Requirements

  • Current Master of Social Work degree from a school of social work accredited by the Council on Social Work Education required.
  • Current Indiana license as a Licensed Social Worker (LSW) required based on state of practice.
  • Minimum of two (2) years of social work experience preferred.

Nice To Haves

  • Case Management Certification (CCM) preferred.
