About The Position

The PRN Pool Social Worker when called upon, helps people cope with the challenges they are facing in their lives. He/she will actively participate in coordinating all aspects of the patient’s care and is a client advocate and adviser. This person also assists with the ongoing interdisciplinary comprehensive assessment, helps develop a plan of care and contributes to educating the patient and their family while giving them counsel as well. This person participates in the QUAPI program and hospice sponsored in-service training.

Requirements

• Must be licensed or license eligible in compliance with jurisdiction of practice (LCSW, LPC).
• Must have a Master's Degree in Social Work from a school accredited by The Council on Social Work Education.
• Two years of master’s level experience in an interdisciplinary health care setting desired.

Responsibilities

• Actively participate in coordinating all aspects of the patient’s care.
