Administration of Human Resource Programs

The Social Worker for the Post 9/11 Military to VA Program provides essential social work services to transitioning and established servicemembers and veterans, along with their families. This role focuses on ensuring a smooth transition from military to civilian life by offering comprehensive support, including psychosocial assessments, treatment planning, and case management. The social worker collaborates with interdisciplinary teams to address the unique needs of veterans, ensuring continuity of care and access to necessary resources.

  • Ensure safe and continuous transition of care for Veterans and Active Duty Servicemembers from DoD facilities to the Veterans Health Administration.
  • Support Veterans, their families, and caregivers by coordinating health needs across agencies and care episodes.
  • Perform clinical interventions and case management services in collaboration with the interdisciplinary team.
  • Conduct high-risk screenings, psychosocial assessments, and develop treatment plans.
  • Advocate for Post 9/11 servicemembers and their families to access needed services.
  • Assist with advance directives, guardianships, and applications for home care services.
  • Participate in training and orientation of social work graduate students and other trainees.
  • Support the mission and policies of the VA and participate in relevant meetings.
  • Must be a U.S. Citizen.
  • Master's degree in social work from an accredited school of social work.
  • Proficient in written and spoken English.
  • Licensed or certified to independently practice social work at the master's degree level for GS-11 level.
  • Ability to work with diverse populations and assess psychosocial needs.
  • Knowledge of medical and mental health diagnoses and treatment procedures.
  • Experience working with veterans and military families.
  • Familiarity with community resources and referral processes.
  • Skills in crisis intervention and problem-solving techniques.
