About The Position
At NewYork-Presbyterian Brooklyn Methodist, the Pediatric Social Worker plays a crucial role in enhancing patient outcomes through compassionate care. This position is dedicated to providing comprehensive discharge planning for patients admitted to inpatient pediatrics and the pediatric intensive care unit (PICU). The Pediatric Social Worker will conduct thorough psychosocial assessments involving both the child and their family, ensuring that all necessary community services are secured post-discharge. This includes arranging for homecare, medical equipment, outpatient mental health services, or inpatient rehabilitation in skilled nursing facilities. In addition to these responsibilities, the Pediatric Social Worker will assist parents in coping with the challenges of hospitalization by providing education, counseling, and access to helpful resources. A critical aspect of this role involves identifying instances of child abuse and neglect, necessitating reports to the State Central Registry. The Pediatric Social Worker will also coordinate transportation needs for patients and collaborate with pediatric psychiatry to develop safe discharge plans. This includes obtaining psychiatric beds for patients requiring inpatient psychiatric treatment, which involves securing insurance authorization, coordinating with families, and finalizing transfers with the NYP Access Center. Participation in interdisciplinary rounds is also a key component of this position, ensuring a holistic approach to patient care.
