About The Position
This position is for a professional social worker within the Partial Hospitalization Program at MaineHealth, located in Portland, Maine. The role involves evaluating the psychosocial needs of patients and their families, providing diagnostic, preventive, and treatment services, which include individual, family, and group therapy. The social worker will also be responsible for complex discharge planning, coordinating aftercare services, and making referrals to other community services based on the treatment plan established for each patient. Collaboration with other health and mental health professionals is essential, as the social worker will provide consultation to the treatment team regarding the social and environmental needs of patients and their families. The work is performed under general supervision, adhering to institutional and social work/clinical counselor policies. Additionally, the social worker may supervise other social workers, students, or staff as needed.
