About The Position

At CHI Health Mercy Council Bluffs, patients are at the heart of our care. Our campus is designed to heal the body, mind, and spirit of every person in a more comfortable, less stressful hospital environment. Our full range of medical services includes a new pavilion featuring enhancements in heart and vascular care, surgery, maternity, cancer care, and diagnostic imaging. We are looking for a Social Worker to join our team and make a real impact for patients and families in the hospital and emergency department. You'll provide support, guidance, and resources, helping patients get the care they need while connecting them to the community services that make life easier.

Requirements

• Master's Degree from a school of social work accredited by the Council of Social Work Education (MSW) REQUIRED

Responsibilities

• Offer crisis support, counseling, and resource referrals for patients and families
• Help plan safe discharges and transitions of care
• Coordinate with healthcare teams to keep patients supported and informed
• Connect patients to community resources for housing, food, transportation, and more
