About The Position

This is a part time no benefits Social Worker position in an Article 31 OMH outpatient facility for one 8 hour shift a week. Located at 184 Court Street this is an active outpatient facility emphasizing a team approach to the best mental health care available.

Requirements

• Masters Degree Non Nursing: Masters of Social Work (MSW) required.
• LMSW, LSW, LCSW is required for staff providing care to patients in the state of New York.
• Minimum two years post graduate experience as a social worker in a similar type of health care setting is strongly preferred.
• Successful completion of a 6 month hospital, based orientation process is required.

Responsibilities

• Supports psycho‐social needs of patients who are currently receiving or recently received hospital based care.
• Enhances the patient’s optimized level of health and/or recovery.
• Provides interventions for populations ranging from infants to geriatrics.
• Serves as a resource for the healthcare team.
• Assists in identifying discharge planning needs.
• Provides support / intervention and resources.
• Provides psychosocial evaluation to meet patient’s medically related social needs.
• Assesses social and emotional factors related to the patient’s illness.
• Participates in unit interdisciplinary conferences to identify patients and families need of intervention.
• Identifies problems and develops plan based on evaluation of patient’s needs.
• Participates in an on‐call system 24‐hour accessibility and accountability to meet patient needs.
• Facilitates discharge planning to meet the patient’s needs for continued care.
• Collaborates with the healthcare team in establishing discharge plan.
• Completes appropriate/required documentation related to transfers.
• Documents appropriately to reflect continuous progress in plan of care.
• Assesses and facilitates financial needs.
• Utilizes various charity funds available in a responsible manner according to policies/procedures.
• Supports patient/family/others during crisis situations.
• Provides interventions for identified high‐risk patients.
• Performs other duties as assigned.
