About The Position

In this position, the Social Worker in the Emergency Department will provide comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. Knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advance directives. In addition the social worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual's health needs. The social worker rounds on their assigned patients with the multidisciplinary team. Additionally, the social worker will participate in quality meetings within their assigned unit. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Requirements

  • MSW from a Social Work program accredited by the Council on Social Work (CSWE).
  • M.S.W., L.M.S.W., L.C.S.W. or eligible.
  • 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment).
  • New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position.
  • Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
