About The Position
The Social Worker in Palliative Care at the New York Center for Dignity and Support is responsible for providing holistic care and services to EmblemHealth members facing serious, life-limiting illnesses. This role focuses on addressing the emotional, physical, and spiritual needs of clients and their care partners through direct services, advocacy, care management, and support. The position involves clinical consultation, care plan development, and community representation, ensuring that clients receive comprehensive support throughout their care journey.
